If you are interested in purchasing, developing, or creating a housing solution for your loved one, consider filling out our form below. CHTFWI works to match families with similar housing goals, and once a family group is formed, they have the option to go through the New Futures Initiative training. This step-by-step process gives families the ability to open doors to supported living options in their neighborhoods. The series of workshops provides the tools, materials, and professional resources needed to move forward on your housing vision.
